FREE ASSISTANCE AND REASONABLE MODIFICATIONS

Oak Lawn Pharmacy provides appropriate assistance in a timely manner and free of charge when needed to ensure meaningful access and effective communication.

When contacting us, describe the assistance or communication method you need.

Do not include unnecessary prescription information, medical information, Social Security numbers, or payment-card information in an ordinary accessibility or civil-rights email.

HOW TO FILE A GRIEVANCE WITH OAK LAWN PHARMACY

You may file a grievance if you believe Oak Lawn Pharmacy:

• Discriminated against you.
• Failed to provide appropriate language assistance.
• Failed to provide an appropriate auxiliary aid or service.
• Failed to make a reasonable modification when required.
• Otherwise prevented meaningful or equal access to a pharmacy service.

A grievance may be submitted in person or by mail, telephone, or email to the Section 1557 / Civil Rights Coordinator.

Please include, when available:

• Your name and preferred contact method.
• A description of what occurred.
• The date or approximate date.
• The service or communication involved.
• The assistance or resolution requested.

Assistance is available to help you prepare or submit a grievance. Reasonable modifications, language assistance, and appropriate auxiliary aids and services will be made available for the grievance process when needed.

Oak Lawn Pharmacy will review the grievance and communicate a response within a reasonable period based on the circumstances.

NO RETALIATION

Oak Lawn Pharmacy will not intimidate, threaten, coerce, discriminate against, or retaliate against a person for:

• Requesting language or accessibility assistance.
• Requesting a reasonable modification.
• Filing a grievance or complaint.
• Participating in a civil-rights review.
• Exercising a legally protected right.

HOW TO FILE A COMPLAINT WITH HHS OFFICE FOR CIVIL RIGHTS

You may also file a civil-rights complaint with the U.S. Department of Health and Human Services Office for Civil Rights.

You are not required to complete Oak Lawn Pharmacy’s internal grievance process before contacting HHS OCR.
